If you are a physician who is not a surgeon and you are interested in becomming an Associate Member of the Michigan Society of Thoracic and Cardiovascular Surgeons, please complete the attached Associate - Physician Membership Application.

If you are a physician assistant, nurse practitioner, perfusionists, clinical nurse or data manager interested in becomming an Associate Member of the Michigan Society of Thoracic and Cardiovascular Surgeons, please complete the attached Associate - Nonphysican Membership Application.
